AHPA Chronic Disease Management Project
Allied Health Professions Australia (AHPA) has been funded by the Department of Health and Ageing as part of the Australian Better Health Initiative to:
- Improve collaboration between Allied Health Professionals and General Practitioners in chronic disease management
- Strengthen working partnerships between allied health professionals and general practice
- Improve client outcomes in relation to chronic disease management via mutual understanding of roles
