The presentation Medicare Items for managing chronic disease was updated in July 2008 to include details of chronic disease related items including those relevant to type 2 diabetes (items 81100 to 81125 and item 713) as well as the item for PNs and Aboriginal Health Workers to provide monitoring and support for people with chronic disease (MBS item 10997). Existing CDM item information has also been updated as relevant.

Interactive templates

AGPN commissioned the development of templates for GP Management Plans, Team Care Arrangements and the review of those plans. See the brief directions for downloading below or push the button for the detailed instructions. The templates are suggestions only, they are not required by the Health Insurance Commission.

	Once downloaded, DO NOT OPEN in Microsoft Word as this will corrupt the templates	1. Right click on template 2. Save the target as a rich text file on your computer Desktop 3. Import into the Medical Director Letter Writer or press button for full directions
