Lifescripts

What are Lifescripts?

Lifescripts are evidence-based tools to support the general practice team to work with patients to address the main lifestyle risk factors for chronic disease, particularly:

  • smoking
  • poor nutrition
  • misuse of alcohol
  • physical inactivity
  • poor weight management.

Lifescripts is a national initiative that provides resources for general practices across Australia.  AGPN and the Network continue to play an integral role in supporting and promoting the uptake of the Lifescripts resources through general practice.

Download a form to order hard copies of the standard, Aboriginal and Torres Strait Islander, and translated Lifescripts resources.

Clinical templates for the Lifescripts resources are available for Medical Director (2) and (3), Best Practice, Practix and Genie.

Pregnancy Lifescripts

AGPN updated the pregnancy Lifescripts, including the development of a new pregnancy Lifescript for physical activity. The revised and updated pregnancy Lifescripts will be available soon.

Indigenous Lifescripts

Lifescripts resources have been developed specifically for use with Aboriginal and Torres Strait Islander patients. The resources can be used by Aboriginal Health Workers, General Practitioners, and Practice Nurses within general practice or in Aboriginal and Torres Strait Islander health care settings.

A number of resources are available for Indigenous Lifescripts including:

  • a patient brochure
  • healthy lifestyle action plan
  • the risk specific tools
  • a flipchart.
