National LMP Facilitator Service Directory

This service directory has been developed to assist you in locating accredited Lifestyle Modification Program facilitators in your local area.

To use the directory, please select your state or territory from the list below. A list of general practice networks within your state will be uploaded. Simply select your local Network Member Organisation and a list of accredited LMPs in your area will be shown. You can then use the contact details provided to contact the LMP facilitator.

If you are unsure as to which general practice network area you are in, you can use the search functions (by postcode or town name) on the network directory or contact your state-based organisation.

Please note that the directory is in the process of being redesigned to be more user friendly and as a result newly accredited facilitators have not been added in their state section. A full list of newly accredited facilitators can be found by clicking here.
