Lifescripts

What are Lifescripts?
Lifescripts are evidence-based tools to support the general practice team to work with patients to address the main lifestyle risk factors for chronic disease, particularly:

  • smoking
  • poor nutrition
  • misuse of alcohol
  • physical inactivity
  • poor weight management.

Lifescripts is a national initiative that provides resources for general practices across Australia.

AGPN and the Network continue to play an integral role in supporting and promoting the uptake of the Lifescripts resources through general practice.

The Generic Lifescripts resources are available as hard copies from National Mail and Marketing.
Order form.

Clinical templates for the Lifescripts resources have also been developed and are available from the Department of Health and Ageing website. The templates are available for Medical Director (2) and (3), Best Practice, Practix and Genie.

Pregnancy Lifescripts
AGPN is currently managing the review and update of the pregnancy Lifescripts, including the development of a new pregnancy Lifescript for physical activity. The revised and updated pregnancy Lifescripts are scheduled to be available in both a hard copy format and electronically as clinical templates in mid 2011.

Indigenous Lifescripts
Lifescripts resources have been developed specifically for use with Aboriginal and Torres Strait Islander patients. The resources can be used by Aboriginal health workers, general practitioners and practice nurses within general practice or in Aboriginal and Torres Strait Islander health care settings.

There are a number of resources available for Indigenous Lifescripts including:

  • a patient brochure
  • healthy lifestyle action plan
  • the risk specific tools
  • a flipchart.

The resources are available as hard copies from National Mail and Marketing: order form
