Mind the Gap - Workshop Downloads

Please find link to a Zipped file which holds templates and documents to be used for delivery of Mind the Gap workshops.  Please read and note the following: 

1. Some of the documents include highlighted text, this is for you to modify and add your own logo, text or details as required – please check everything prior to sending to your network as it may need to be modified.

2. The project evaluation documents (pre/post/follow up workshop questionnaires), as detailed within the “Evaluation Guidelines” spread sheet, will be sent to you separately.  Please contact Carol if you have not recieved these.

3. Facilitator and participant manuals are included as PDF files for reference and distribution to your facilitator/s as required.  You will be sent the hard copies closer to your scheduled dates – please follow up if you have not heard from AGPN regarding your participant numbers and timeframes.  We require at least 2 weeks’ notice to organise manuals to be delivered to you. 

4. You will note a copy of the “Service mapping form” – you will need to print & have copies available for participants during the workshop.

5. Please advise as soon as possible when you have your workshop scheduled (date & location) as we will upload details to the RACGP online education calendar.  This will mean you may capture additional GP participants as they search for training and workshops available within your area.
